Do i need a referral
Warren Pain Clinic and Chautauqua Pain Medicine works with your primary care physician. All new patients will need a referral from their physician to obtain an appointment.
Once a referral has been received and insurance is accepted we will schedule an appointment for you to see
Dr. Kourtu or Wednesday Elder, CRNP as soon as possible.
Your doctor's office can contact us at (814) 726-7365 to learn about our referral process. In most cases we will need a current (within the last year) MRI prior to seeing you.
